what happens when a question word is subject of a question in the present simple? would need a few exaples

What do you mean, "What happens?"

Can you provide an example of the kind of question you are talking about?

Here's an example:

So, maybe the author ment things like:

— What is inside?
— What or Who? — meaning one may expect a living creature there.

The difference relies mainly in questions.

When a question word is the subject of a sentence it won't need an auxiliar verb to make a question,

eg: Somebody came into the house

Who came into the house?

but when the question word is the object of the sentence it will request an auxiliar in questions.

eg: Joseph saw somebody coming i
