Your sentence might be said by a native speaker, and is perfectly intelligible, but it is not great English in my opinion.

Almost always you should use "What happened?". "What did ..." is normally used when asking about the object of a transitive verb (e.g. "What did she say?"), which is not the case here. However, "What did happen?" is possible in the case where you want to emphasise "did", for example after some long discussion which doesn't actually get to the point.
