What does "Where's your head at?" mean?

I was recently asked the following questions by my American manager:
"Where's your head at? How are you mentally?"

That didn't sound friendly to me. Another American colleague of mine says the questions are not intended to offend. They just mean - "How are you doing?". I'm not convinced. I'd like to hear from you guys what it means when someone asks me- "Where's your head at?"

It can be derogatory, depending on the tone. eg.
What in the world are you thinking when you do this (stupid) action?
