What does this sentence mean



Has any person applying for coverage ever been seen, treated, hospitalized, or had surgery for a bypass, angioplasty, stent, aneurysm, valve replacement, cancer, stroke, gastric or weight loss surgery, congenital abnormality, or organ transplant other than indicated elsewhere on this application?...................................................................................................................

Is the intent in the sentence above to just find out if the person has ever been treated for the listed conditions only?
