"What does it means?" is ungrammatical - incorrect.

In question form, the auxilliary "do" is inflected, not the main verb.

In the statement form ( It means what), the main verb "to mean" agrees with "it."

In the question form, the main verb takes the base form, "mean," and the auxilliary agrees with "it." (It does.)
