What do they mean?

When somebody says "She is a qualified accountant", what do they mean? "She is an accountant who is qualified for a certain task" or "She is a CPA" or something else?

Yes. The task is determined by those who are hiring her.

So "qualified" means "qualified (i.e., competent) to do what is necessary for the job in question".
