A applies to #1, although without further context I would take the sentence to mean that John did get angry.
I don’t find the other three very natural. These are better:
What did Pete say that made John angry? (Pete indeed angered John but not necessarily on purpose.)
What did Pete say so that John would get angry? (Same as your first sentence.)
