What are the differences?

I'm quite confused with the usage of singular and plural in the following sentences. So someone please explain how these differ in their meanings.

* I have a back pain. vs I have back pains.
* I have a pain in my back. vs I have pains in my back
* I have a muscle ache. vs I have muscle aches.

Hi Anon, if plural is used, we are talking a bout a "general" state, we ususally or always have back pain, muscle ache etc. che[H]ers
