Were running on empty

What does it mean "were running on empty"?

What is "fully up"? thanks

Our local hospitals were running on empty. Patients were staying on trolleys in corridors. I tremble to think what the death rate for pensioners would have been this winter if that version of Thatcherism had been fully up and running this year.

Running on empty means that you have hardly any petrol in the tank, and the fuel gauge is pointing at "Empty". Here it means that the hospitals had only just enough resources to keep going. Fully up and running means functioning just as it's supposed to , completely operational.
