I think only the second one is possible.

1. A: "Pieanne, you really don't have brains!" - Pieanne "No, (you're right) I don't (have brains)"

1. A: "Pieanne, do you have brains?" - Pieanne: "No, I don't (have brains)"

But maybe someone will say it's not true!

I don't know the context of that answer, Jr Danny, but I find it very strange to say "YES, I do NOT have something"...

2. Person 1: " I think you have brains" or "Do you have brains?"

Person 2: No, I don't have brains"

Yes and No are used to show agreement/disagreement with
