Was or were

Successful completion of a thesis, theoretical and practical exams ____ required to obtain the diploma from the American Bankers Association.

I cannot decide: WAS or WERE?

There's something wrong with the sentence. Maybe:

Upon successful completion of a thesis, theoretical and practical exams were required to ...

Or:

Successful completion of a thesis and theoretical and practical exams were required to ...

It's were in either case because the subject is plural.
