"was" or "had been"?

It's going fine, though there was/had been an incident about two weeks ago. One of the other girls (had) pulled her hair. One of the nursery assistants working there told me about it when I was there to pick her up.


If you were to say this to someone, how would it sound like...

Would you say "was" or "had been"? And would you say "had" or leave it out?

Just use Simple Past to describe a simple past event. The sequence of events is pretty clear heer.
