Dear Sir/ma'am,

This is to inform you that on the 31st of March and the 1st of April, I will need permission to attend "my elder sisters marriage" and "my mother, grand mum, my elder brothers memorial service" and some family issues. I will be so happy if my permission is granted.

In the US, there is no possibility of saying "Your faithful servant." If that is culturally acceptable where you are, you may use it. I would never use it. Be sure to say "Your" not "Yours" if you use it.
