The first isn't a question: "I don't know what she wants."
The second is.

Use (a) as part of another sentence: Nobody knows what she wants. / Does anyone know what she wants?

Use (b) when it stands alone or in a direct quote: What does she want? / "What does she want?" asked Bill.
