voiced /t/ vs flap /t/

I've read that a voiced /t/ and a flap /t/ are not the same thing. Is really a difference? I'd say a voiced /t/ is really a /d/, and a flap /t/ an /?/.
Am I right? If so, are they being used in real English (especially in Britain)?
