"Very good!" or "Very well!" as feedback

I heard it's incorrect (or only uncommon) to say Well/Very well as a feedback e.g. to a child/student answering a question in a class. Is it so?

how about it's children playing and performing indicated activities, following instruction (no question) - is it ok to say "very good!"? I presume "very well" never fits in any context like that.
