Verb Tense - Accounts or Accounted / Article Needed

Although there are many medical terms here, my problem is confusion with what verb tense to use for "account" - accounts or accounted and also if "minor vascular injury" needs an article in the following sentence.

Since the intra-abdominal abscess was far from the pseudoaneurysm, minor vascular injury during the excision of the xiphoid process most likely accounts for its formation.

Accounts or accounted -- Both are OK. It can be looked at either as the past cause or the present diagnosis. An article is unnecessary with injury , which can be uncountable, and here is better so, I think.
