Authors: The author of this article is Michele Keller. She is a register nurse and has a BSN in nursing. She graduates with Diplomatic Nursing Science, Restaven Inc, Paradise, South Australia. The author has a sufficient knowledge about the art of nursing that qualify to conduct this research due to the fact that she have achieved a degree in nursing. Audience: The primary target audience of this article would be nurses working in residential care and nursing homes and nurses who care for patients and are responsible for maintaining hydration in older patients. Purpose: The purpose of this article is to improve the hydration status of Nursing home residents and to prevent being at risk of dehydration in older patients. The problem that is expressed in the article is that older adults are susceptible to dehydration because of age-related changes such as decreased renal perfusion and sensitivity to antidiuretic hormone (ADH). The sense of thirst is reduce, even in healthy older adults and combined with mobility problem, confusion and pathological conditions such as hypertension, heart and renal disease making older people highly sensitive to fluid and electrolyte imbalance (Keller, 2006). The methods that were used in this article were by conducting a research on population of dependent and semi-dependent people over 65 years of age from six of the Resthaven residential care sites. There are total of 229 residents that participate in the project and they were monitoring their fluid intake and assessment of risk of dehydration. The process and population of residents were entered into the program called Application of Clinical Evidence System (PACES). Data Collection ensures that the privacy and confidential of the residents are maintained, there are no identifying details of the patients were entered into the program (Keller, 2006) Evidence: The study reported that dehydration has been the most common fluid and electrolyte imbalance in older adult. Older people are being at risk of dehydration due to age-related changes including many pathological conditions that may impact on drinking ability. The main concern of the Department of Health and Ageing was about inadequate fluid intake of residents in the high care facilities. According to the article, fluid intake for all residents were not routinely monitored and it only monitors in general or clinical observation identifies need (Keller, 2006). Nutrition and Hydration standard require having procedures that assess, document, manage and review resident hydration and identification of deficits including physical, psychological and behavioral factors that may impact on drinking ability. The article made references to the JBI-BPIS ‘Maintaining Oral Hydration in Older Adult', that individual resident at risk of dehydration have fluid intake measured daily and should consume 1600mL daily oral intake. Conclusion: After reading this article, it helped to understand more about fluid volume deficiency among patients in nursing home. I learned how important to maintain a normal balance between the body's fluids and electrolytes for a person to be healthy, especially in older adults. Older adults are in high risk of dehydration due to age-related changes that impact they drinking ability, so it is important for nurses to keep them hydrate and healthy. In a clinical setting I will be more aware of patients that risk of dehydration. I will teach the client and family about fluid and electrolyte problems, especially of the person will be cared for in the home. I will also explain the important to keep the fluid and electrolytes balance. I will encourage patients to drink adequate amount of fluid daily. I will also monitor client's input and output closely and monitor any signs of dehydration such as skin's appearance and skin turgor, if any changes occur I will notify the primary nurses.
