Vacation Leave Letter This Coming Holidays

I'll be taking a vacation leave this holidays and it is really important that I'll be leaving on the specify day I have put in my letter, so how do you say it or put it in a letter in a professional way and not in a rude or forceful? And I think, my letter is a bit off (especially the introduction).
All of your help will be appreciated. And it's ok if you'lll edit my whole letter, as long as the message or my point in my letter is still intact. Thanks a lot.
