Vacation Leave Application

Please Help....

Dear Sir,

I wish to go on my vacation from August 4, 2013 till August 31, 2013 and shall resume back to duty on September 1, 2013.

html I would have at least written "I would like/I am planning on taking a vacation" instead of "I wish" "Will be back on duty"/"will be back in the office". You also need a comma after the year.
