Using 'yourself'

I am trying to correct a questionnaire. The person who has authored it has used 'yourself' and I am sure it is incorrect.
The question is
1) Who brought up the topic of long-term prognosis during your appointments? (Tick all that apply)
a. Yourself
b. GP
(.... more options follow)

the answer 'yourself sounds wrong to me, and I believe it should be 'you'. If anyone could please advise, I would be most grateful.

I would use You here. Yourself is reflexive, which means you need a subject to refer back to. You did it to yourself.
