Using the verb "help"

I think we can use "help" like...
1) help + do
2) help + to do
3) help + doing

Are these correct?

If these are correct, how are the expressions different?

Only these are correct:

Can you help me do it?
Can you help me to do it?

Passive: I was helped to do it.
