Using identations properly when writing a novel.

Within a paragraph, assumming 2 or 3 people are talking, plus using some descriptive dialogue, please answer the following question:

1. Should a new line & a new indent start when another person starts talking?

2. Should a new line start when descriptive dialogue begins & should it be indented?

3. If a person talking is followed by or preceeded by dialogue, is it proper to sandwich the quoted person within the dialogue, or should the quotation start a new line with indentation?
