Using article twice

It was a thin man in a blue shirt.

Does it sound clumsy when 'a' is written twice, before 'thin' as well as 'blue'? Do we normally say 'in blue shirt' or 'in a blue shirt'? I seem to recall reading it both ways. The woman in nightie, the man in blue suit etc. etc.
