Use of the word mind

" Do you mind if I open the window ? "

" Would you mind if I opened the window ? "

Which of the above two sentences is more formal ?

I don't often hear the word "minded" as the past form of "mind".

Would it be proper to use the word " minded " in a sentence as in "She never minded how hot it was."?
