Use of period

"I have been seen many times for acne, acne scars and other services. I truly appreciate the wonderful work and the staff" M.B. (45 Y.O.)

Is the format used in "M.B. (45 Y.O.)" correct? Should there be a space between the period as in " M. B. (45 Y. O)?

Note : The reviewer does not want his name and surname to be published, hence the first letter use.

By Y.O., I think you mean 'Years Old'

I'd say M.B. (age 45) or just M.B. (45)
