use of entitle in this case

which is the correct use of the verb entitle in this context:
"I am the person to whom this security code has been assigned and entitle to enter." or "I am the person to whom this security code has been assigned and entitles to enter"

Neither of those is correct. This is possible:

"I am the person to whom this security code has been assigned, and who is entitled to enter it."

It wold be more natural to say simply
eg This is my security code. Only I can use it.
or just
eg This is my security code.
