Use of 'Can I ' ???

I would like to ask some questions about the usage of 'Can I'

As far as I know, we use 'can I' when we order something at a restaurant and request something.

But, in some situations, I feel kinda awkward.

For example, when I'm at a bank and ask the staff to put a waiver on my account

can I say

1. can you put a waiver on my account??

2. can I put a waiver on my account??

3. I would like to put a waiver on my accout.

which one is right???

What usually comes up is "can" (to be able) vs. "may" (to be permitted.)

The one that bugs me is, "Can I get a glass of water?"
I want to say, "Sure! Go for it!"

In polite society, "May I please have X?" is usually considered preferable to "Can I have X," even in a restaurant.
