Usage of the word 'present'

'They can give birth with an obstetrician present'

I have seen the sentence and I was wondering if present modifies obstetrician behind like 'obstetrician (who is) present' or the structure is like 'with my mouth open' and it implies 'while an obstetrician is present'? I think that the second is more natural or both are possible and there is no meaning difference either way?

What do you native English speakers think? Thank you so much as usual in advance.
