Usage of Affiliation

Hello!

I am not a native English speaker but I'm an advanced user. I am filling a form and I came across this section which says Affiliation:____. This is about references so it has the email, name of the person (my lecturer) I have as a reference and Affiliation field. So the question is: should I write 'Lecturer' or 'Student' (for me being his/her student) or both are correct?

Thank you!
