Understand

"I didn't understand the word."

Can this mean that I didn't understand what the word was on account of the accent as opposed to that I don't know the meaning of the word ?

If it were the matter of sound, he should say, "I couldn't hear the word (it, you)."

It sounds like he didn't understand the word itself or why that particular word was used. (It could have left the sentence unclear).

I didn't understand a word - that would be where the speaker's voice was unclear to the listener.
