Two tricky (for me) parts of the sentence!

On July 5, 2011 I received a letter No. 134 9 from Social Services of July 3, 2014 with a request to provide psychologist's services in order to determine John's opinion and to provide a conclusion and answers on the following questions interested by the Social Services: /and then there is a list of questions/

Hello!
Both underlined parts worries me. First underlined part should mean that the letter was written on July 3, 2014 (although I received it on July 5). The second underlined part should mean that the further list contains questions for which the Social Services wishes to get answers.

Maybe there are other mistakes as well?
