Two parts from an article in META

Dear teachers,

Meta is the name of a translation journal.

I have two questions about the redly underlined parts of the below picture:

1) What is/are the author's name and surname? (My guess: name: Jamal. Surname: Al-Qinai?)

2) What is/are the volume and issue respectively? (The smaller picture is from the left-lower corner of the first page of this article. If you want to see the article, you can download it at the publisher's website and here is the direct link: http://www.erudit.org/revue/meta/2000/v45/n3/001878ar.pdf )

Any comments are welcome! Many thanks in advance!

(One more question: Do you think I need is or are in my questions?)

Hi,

1) What is/are the author's name and surname? (My guess: name: Jamal. Surname: Al-Qinai?)

Particularly since you have used the term surname, it's clearer to say that his first name or given name is Jamal.
