Two "Is's" and "Then" comma

First question:

"What it is, is ridiculous."

Doe there need to be a comma between "is" and "is" to make it easier to read?

Second question:

"Why not, then?"

Does there need to be a comma before "then" and does that apply to all sentences that end with "then"?

Is there ever a case where you put a comma between two of the exact same word for simple fact that it's the exact same word?

Not that I know of, no. If the double is bothers you that much, you could rearrange the words: "Ridiculous is what it is."
