"Why?! It'll just be more pretentious nonsense," he exclaims.
Is it okay that there isn't an exclamation mark after "nonsense," instead of a comma, before it's made explicit that he's exclaiming? It's not inconsistent with an exclamation mark after "Why?," is it? I've seen commas and I've seen exclamation marks before something like "he exclaims," so I don't know what's better or if it even matters.
Thanks.

I suppose it doesn't really matter. I'd just use the comma.
