Two Commas and a "That"

Please only respond to me regarding the first question if you use adjectival serial commas in the first place:

She sat on the black, leather couch he had against the wall, across from his desk.

Is that comma between "black" and "leather" right? Also, how is that last one before "across"? Finally, is the lack of the word "that" before "he" awkward?
