Twisted up

Hello.

In the Goldbergs opening song, it says: "I'm twisted up inside, but nonetheless I feel the need to stay."

Does twisted up here mean mentally unstable?

Thanks in advance.

Yes, something like that. I might say "full of anxiety" or "having a distorted personality". It's slang and therefore not clear.
