Twist in his/her lap

I've seen the phrase "twist in his/her lap". But I didn't know how to visualize this phrase. Does this mean your lap twists??? But it sounds kind of painful...
What is the motion of "twisting in his/her lap"? What do you do actually when you twist in your lap?

If I understand this, the lap does not twist. It's the hands/fingers which twist. I don't believe it implies that the hands/fingers engage the lap in any way.

If I understand this, the lap does not twist. It's the hands/fingers which twist.

I don't believe it implies that the hands/fingers engage the lap in any way.

People often sit with their hands "folded" in their laps, or with their fingers laced together.
