Twist

My cousin told me, 'Yesterday I met a patient in hospital and I'm not sure if he was smiling or his face got twisted just that way due to paralysis.' Although I couldn't find any fault with the sentence still I have doubt. Is the sentence grammatically correct?

Largely this is OK, though "had got" would be better in my opinion, and the word "just" doesn't really seem necessary, or is misplaced (should be before "got"?).
