Top off

Hello,

what does "top off" mean in this sentence?

You are rigid in the chair, hands clenched, soaked with sweat—and then the dentist leans over and says, “We’re done now. You can go home. But if you want, I’d be happy to top you off with a few minutes of mild pain.”

Thank you

'Top off' means 'completely fill', 'completely satisfy'. We top off our gas tank at the gas stand by pumping the last few centiliters of gas into it; we top off a delicious meal with some delightful dessert; or we top off a terrible weekend by oversleeping Sunday night and getting to work late Monday morning.

It's a reference to the fuel tank in an automobile. The Brits would say "top you up". When you want to fill your tank, but it will take only a small amount of fuel to do it, you are topping it off. This can be said about any liquid in any container.
