TOEIC/ definite, indefinite articles

After receiving (A) a hospital bill, the patient questioned (B) an error in (C) the charges and received (D) the reimbursement from the hospital.

According to the answer key, (D) is incorrect. I think reimbursement is a count noun and a general subject matter, so I would correct it to a reimbursement. Is that right?

I also wonder why not use the hospital for (A) because I think it can refer to a specific item . Please advise.

There are a number of possible permutations here that are correct.
I think the examiner wants you to correct D to a reimbursement, or the more general reimbursement.

As regards the phrase 'a hospital bill', I think the examiner wants to talk about a bill that has not been previously mentioned, and thus does not want you to change 'a' to 'the'.
