TOEFL/Cambridge ESOL/Council of Europe

Hello!
can anyone explain to me what relationship is between TOEFL, Cambridge ESOL and Council of Europe examination results? I mean, if I have FCE certificate, can it be used as equivalent of TOEFL, and how this should be calculated?
