I have made up the sentences below.
(1a) John is the only one in our family to wear glasses.
(1b) John is the only one in our family who wears glasses.
(2a) Bill is the only one to have won five gold medals in the tournament.
(2b) Bill is the only one who has won five gold medals in the tournament.
