I always see that to have v3 form is used after base verb (it seems to have reached) i really don't know when it is used

By 'v3', do you mean the past participle, as in eat, ate, eaten? The past participle is used in two verb forms:

The perfect aspect: I have seen a ghost. I had seen a ghost when she called.
The passive voice: The ghost was seen by everyone. Sushi is eaten in Japan.

I mean , I know where to use present perfect tense and past perfect tence.. but i really don't know what it means (it seems to have reached) why do we use 'to have v3' after the base verb?
