To be or not to be, that is the question!

Hello

I am writing a report that should include a title in the form:

"Documentation Needs to be Improved"

However, is it gramatically correct to say:

"Documentation Needs be Improved"

If both are correct, which one is more chic?

Oh, I'm sorry. I should've been clear that the phrase is going to be used as a heading to an issue within the report and not as the title of the report.
