The pupils of St Theresa School have nominated two of their teachers, Mr Lim Eng and Miss Teo Susan to be / as / are the most popular teacher of the year.

The pupils of St Theresa School have nominated two of their teachers, Mr Eng Lim and Miss Susan Teo for the most popular teacher of the year. I presume that there is only one most popuar teacher? Remember that in English the personal name comes first and the family name follows.

Should I know, why don't we say "to be / are" as the answer??

The verb is 'nominate' and the collocation is 'nominate for'. 'Nominate to be' is OK, I suppose.
