Titles and abbreviated titles for physicians

Hello! I'm a publicist who is trying to write a biographical sketch for a U.S. physician. Because I have a 150 word limit for this bio, I'm wondering if there is perhaps any accepted abbreviation for diplomate affiliations. Specifically, this doctor is a Diplomate of both the National Board of Medical Examiners and the American Board of Orthopaedic Surgery. Is there any acceptable method to abbreviate these -- i.e., acronyms such as fellowships like FAAOS [Fellow, American Academy of Orthopaedic Surgeons]? If so, is it also appropriate to put these diplomate acronyms after the doctor's name like the fellowships -- e.g., John Doe, MD, FAAOS, FACOG?

Any assistance/advice anyone would be willing to give me would be GREATLY appreciated.
