How do you say this? The man wanted to become the chief of surgery.

Must we capitalize chief and surgery. Must we avoid 'the'?

I don't think that you have to capitalize either chief or surgery even though I wouldn't be surprised if someone did. Your sentence requires the article, however I would also accept The man wanted to become Chief Surgeon. (no article but capitalized) or The man wanted to become Surgeon-in-Chief.
