Time, Days, Dates and Time

Hi all-

I am wondering how to properly phrase a calendar I am going to send out at work. See below:

"Accurate Electrical to complete electrical rough-in's at 178 Willow Street on Thursday, December 11th, 2014 at 4:00pm."

Have I inputted the day, date, and time in the correct order? Is it grammatically correct? I would appreciate any criticism and help.
