Three quick questions

Hello,

I have two quick question. First, what is the correct/polite order to use object pronouns. Which one is correct: "Please give the documents to Tom and me" or "Please give the documents to me and Tom."

Second, is the comma placed correctly in the following sentence: "I work in Smitsville, which is located fifteen miles outside of Oakdale."

Finally, what is the grammar/writing rule regarding writing out numbers. As a child, I remember one my grammar teachers telling me that you only spell out numerical amounts (i.e. one, two,three) for numbers below ten. Anything beyond ten, you can write out numerically (i.e. 12,500). Is this correct?
